Wie den Grauen Star behandeln? Laser oder Ultraschall?

Eine Katarakt wird behandelt, indem die trübe, natürliche Linse entfernt, und durch eine künstliche Intraokularlinse (IOL) ersetzt wird.

Die Operation

Operation mittels Ultraschall

Der erste Teil des Eingriffs besteht darin, mit einem speziellen Messer drei kleine, selbst-schliessende Schnitte in die Hornhaut zu setzen. Durch diese führt der Chirurg die chirurgischen Instrumente ins Auge ein und führt Eingriff durch. Dabei wird die alte Linse entfernt und die neue IOL eingesetzt wird. Die alte Linse wird durch eine körpereigene Struktur, den so genannten Kapselsack, an ihrem Platz gehalten. Der Chirurg eröffnet ein kreisrundes Loch in den Kapselsack, um (1) die Entfernung der alten Linse im Inneren zu ermöglichen und (2) die neue IOL im Kapselsack zu fixieren. Das kreisförmige Loch kann manuell erstellt werden, indem der Chirurg den Beutel ergreift und eine runde Form reißt.

Seit Jahrzehnten wird bei der konventionellen Kataraktoperation die alte Linse mit Hilfe von Ultraschallenergie, die von der Spitze eines der kleinen chirurgischen Instrumente abgegeben wird, zertrümmert (ein Verfahren, das "Phakoemulsifikation" genannt wird). Die Fragmente der alten Linse werden dann abgesaugt. 

Risiken

Die manuelle Kataraktoperation ist eine der am häufigsten durchgeführten Operationen in der Medizin. Dennoch handelt es sich dabei um einen delikaten Eingriff, der vollständig manuell durchgeführt wird, was bedeutet, dass der Chirurg eine sehr ruhige Hand benötigt. Die größten Risiken bestehen in der Erstellung des Lochs im Kapselsack und in der Fragmentierung der alten Linse. Sehr selten kann es aber zu Komplikationen wie einer Netzhautablösung kommen, welche eine Netzhautoperation erforderlich macht.

Operation mittels Femtosekunden-Laser

Seit 2011 ist es möglich, einen Femtosekundenlaser bei einigen Schritten der Kataraktchirurgie einzusetzen. Der Laser ist sogar noch präziser als die ruhigste Hand, da alle Schnitte im Voraus am Computer geplant und genau an der vom Chirurgen gewünschten Stelle platziert werden können. Mit dem Laser können die ersten Schnitte in der Hornhaut, das Loch im Kapselsack und auch die Zertrümmerung der alten Linse vorgenommen werden, wodurch die Phakoemulsifikation einfacher und schneller durchgeführt werden kann. Diese Kombination bietet den Patienten den größtmöglichen Nutzen aus der Optik der IOL und die größtmögliche Sicherheit. Unter bestimmten Umständen ist der Laser daher der konventionellen, manuellen Kataraktchirurgie überlegen.

Risiken

Die Risiken sind gegenüber der herkömmlichen Methode geringer, da die empfindlichsten Teile des Eingriffs mit der Präzision eines computergestützten Lasers durchgeführt werden.

Overall as a 50 year old, who wanted to continue to avoid wearing glasses and/or contact lenses, my goal was 100%... achieved with ELZA and Dr. Hafezi. I greatly appreciated the overall approach by Dr. Hafezi and his friendly and competent team in reviewing my vision at the start and following a comprehensive eye exam- the explanations of my options. For me I chose PRK in one eye for a monovision solution to correct short sightedness which was becoming more prevalent in night driving and sports such as skiing and cycling. PRK heals slowly but the outcome is worth the wait- and the cost- you use your eyes every waking moment- and for me I cannot see any reason not to pursue better vision with the ELZA team! I have been to the ELZA clinic to have surgery for my miopy, as my eyes were rejecting the contacts and I was tired of... wearing glasses. From the first preliminary visit, to the actual surgery and post-surgery care Dr. Hafezi and all his collaborators have been extremely professional, considerate and helpful. All the steps of the (very short!) surgery were thoroughly explained to me in order to be as calm as possible, together with the medication plan and precautions to be aware of so that my eyes could heal in the best way possible. My left eye had to be operated on because of a cataract. As I had an Artiflex lens implanted a few years ago, it had to... be removed first before a modern lens could be implanted to correct the cataract.
