Os primeiros segmentos de anel sint\u00e9ticos eram feitos de polimetilmetacrilato, ou PMMA. Embora estes implantes possam remodelar a c\u00f3rnea, o material sint\u00e9tico tem limita\u00e7\u00f5es, incluindo complica\u00e7\u00f5es relacionadas com o tecido e redu\u00e7\u00e3o da integra\u00e7\u00e3o fisiol\u00f3gica.<\/p><p>Segmentos Intrastromais de Anel Alog\u00e9nico da C\u00f3rnea, ou CAIRS, utilizam tecido de c\u00f3rnea doado em vez de material sint\u00e9tico. Esta abordagem foi concebida para melhorar a biocompatibilidade e a integra\u00e7\u00e3o tecidular, permitindo a difus\u00e3o atrav\u00e9s do tecido implantado. No entanto, os segmentos de c\u00f3rnea doada s\u00e3o naturalmente moles, o que pode dificultar a sua inser\u00e7\u00e3o em t\u00faneis estromais durante a cirurgia.<\/p><h4>O que o novo estudo do JCRS examinou<\/h4><p>O novo estudo investigou se a reticula\u00e7\u00e3o da c\u00f3rnea com ultra-alta flu\u00eancia poderia aumentar a rigidez do tecido da c\u00f3rnea alog\u00e9nica antes do implante. Esta abordagem \u00e9 conhecida como <a href=\"http:\/\/www.elza-institute.com\/pt\/ceratocone\/eco-cairs-keratoconus\/\">ECO-CAIRS<\/a>, que significa Segmentos de Anel Intrastromal Alog\u00e9nico Corneal Extracorporal.<\/p><p>Neste estudo experimental ex vivo, 84 c\u00f3rneas su\u00ednas foram divididas em quatro grupos. Um grupo serviu como controlo n\u00e3o tratado, enquanto os outros grupos receberam cross-linking da c\u00f3rnea com flu\u00eancias totais de 10 J\/cm\u00b2, 30 J\/cm\u00b2 ou 60 J\/cm\u00b2. Os investigadores mediram ent\u00e3o a resposta biomec\u00e2nica do tecido da c\u00f3rnea utilizando extensometria de tens\u00e3o-deforma\u00e7\u00e3o.<\/p><p>O estudo concluiu que a reticula\u00e7\u00e3o aumentou a rigidez do tecido em compara\u00e7\u00e3o com o tecido de controlo n\u00e3o tratado. Os grupos de ultra-alta flu\u00eancia, tratados com 30 J\/cm\u00b2 ou 60 J\/cm\u00b2, apresentaram maior rigidez do que o grupo de alta flu\u00eancia convencional de 10 J\/cm\u00b2. N\u00e3o houve diferen\u00e7a significativa entre os grupos de 30 J\/cm\u00b2 e 60 J\/cm\u00b2, sugerindo que 30 J\/cm\u00b2 pode j\u00e1 proporcionar otimiza\u00e7\u00e3o biomec\u00e2nica substancial neste contexto ex vivo.<\/p><h4>Por que isto \u00e9 relevante para o ECO-CAIRS<\/h4><p>As descobertas apoiam o conceito de que a reticula\u00e7\u00e3o extracorporal de ultra-alta flu\u00eancia pode tornar os segmentos de anel da c\u00f3rnea alog\u00e9nicos mais r\u00edgidos antes da implanta\u00e7\u00e3o. Em termos pr\u00e1ticos, isto pode melhorar o manuseamento cir\u00fargico, tornando os segmentos mais f\u00e1ceis de inserir e posicionar no estroma da c\u00f3rnea.<\/p><p>Como a reticula\u00e7\u00e3o \u00e9 realizada fora do olho, as preocupa\u00e7\u00f5es habituais in vivo relacionadas com a seguran\u00e7a endotelial e a nebulosidade p\u00f3s-operat\u00f3ria s\u00e3o menos relevantes. O estudo observa tamb\u00e9m que o aumento da rigidez pode ajudar a reduzir o incha\u00e7o do segmento e a melhorar as propriedades de manuseamento do tecido durante a cirurgia.<\/p><h4>Um passo na biomec\u00e2nica translacional da c\u00f3rnea<\/h4><p>Este estudo acrescenta evid\u00eancias biomec\u00e2nicas ao desenvolvimento do ECO-CAIRS como uma abordagem avan\u00e7ada para a reabilita\u00e7\u00e3o do ceratocone. O trabalho permanece experimental e ex vivo, o que significa que foi realizado em tecido laboratorial e n\u00e3o numa coorte de pacientes em contexto cl\u00ednico. S\u00e3o necess\u00e1rios mais estudos cl\u00ednicos para avaliar o comportamento a longo prazo do segmento, a integra\u00e7\u00e3o tecidual, a altera\u00e7\u00e3o da forma da c\u00f3rnea e os resultados visuais ap\u00f3s a implanta\u00e7\u00e3o.<\/p><p>Para os pacientes, a mensagem principal \u00e9 que o tratamento do ceratocone continua a evoluir para al\u00e9m da simples estabiliza\u00e7\u00e3o da doen\u00e7a. Em casos selecionados, a cirurgia corneana moderna pode tamb\u00e9m ter como objetivo melhorar a forma da c\u00f3rnea e a qualidade \u00f3tica.
