Warum transPRK
statt LASIK?

transPRK is safer than LASIK.

Die moderne berührungsfreie transPRK ist nicht vergleichbar mit der ursprünglichen PRK-Methode. Zudem ist heutzutage eine transPRK sicherer als eine LASIK.

Vor fünfzehn Jahren hat unser Chefchirurg Prof. Farhad Hafezi bei Lasereingriffen in 90% der Fälle eine LASIK und in nur 10% der Fälle eine PRK durchgeführt. Heute ist dieses Verhältnis genau umgekehrt. Wir können (und tun) beides mit den neuesten Geräten, da wir über die Technologie sowohl für transPRK wie auch für FemtoLASIK verfügen. Warum dennoch diese Veränderung?

Die Antwort ist einfach: Wissenschaft. Die transPRK unterscheidet sich heute sehr stark von der Technik, die Anfang der 90er Jahre als “PRK” bekannt war, und unser Verständnis um die Hornhaut – und was die lasergestützte refraktive Chirurgie mit der Hornhaut macht – ist heute viel weiter fortgeschritten. Wenn es um ein tiefes Wissen um diese Veränderungen geht, haben wir das Glück, mit Prof. Hafezi einen Chirurgen im Team zu haben, der nicht nur die Operation in den letzten 2 Jahrzehnten begleitet hat, sondern auch eine Forschungsgruppe für Zellbiologie des Auges an der Universität Zürich leitet, was bedeutet, dass er Einblicke von den “Molekülen bis zum Menschen” hat.

Betrachten wir einige der Mythen, die sich um die moderne PRK ranken.

Mythos 1:
eine PRK verursacht wesentlich mehr Schmerzen als eine LASIK

Einer der Vorteile der LASIK gegenüber der PRK war, dass sie in den ersten Tagen nach der Operation weniger Schmerzen verursachte. Heute ist das ist nicht mehr der Fall dank der Fortschritte in der modernen Schmerzbehandlung. Wenn wir 24 Stunden nach der Operation unsere Patienten sehen, berichten diese entweder von «sehr wenig» oder «gar keinen» Schmerzen. Wichtig ist auch, dass die moderne transPRK die Hornhaut nur mit Laserenergie behandelt – eine Klinge wird nicht mehr eingesetzt («bladeless surgery»).

Mythos 2:
Nach einer PRK dauert es Wochen, bis die Sehkraft ansteigt.

Dies ist nicht mehr der Fall. Unser Excimer-Laser SCHWIND AMARIS der neuesten Generation ist den Lasern der frühen 2000er Jahre um Meilen voraus. Die Technologie hat sich in Bezug auf die Ablation der Hornhaut so weit entwickelt, dass eine transPRK mit dem AMARIS eine wesentlich glattere Oberfläche hinterlässt, die weniger Entzündungen verursacht und zu einer schnelleren Heilung führt. Das bedeutet auch, dass die visuelle Regeneration wesentlich schneller verläuft als bisher: typischerweise sehen unsere Patienten bis zu 60% am zweiten Tag und erreichen oft sogar 80-100% am vierten Tag.

Die transPRK ist genauso effektiv wie die LASIK bei der Korrektur Ihres Sehvermögens (es wird der genau gleiche Excimer-Laser verwendet). Aber der wichtigste Grund, warum wir die transPRK bevorzugen, ist die Tatsache, dass sie eine stärkere Hornhaut hinterlässt. It’s all about safety. Und eine transPRK ist sicherer als eine LASIK.

Tatsache: eine transPRK ist sicherer als eine LASIK.

Dies kann man nicht wegdiskutieren: wir haben die Ergebnisse der Experimente, die dies beweisen, durchgeführt und veröffentlicht (siehe unsere Beiträge am Ende dieser Seite). Eine transPRK hinterlässt die Hornhaut in einem weitaus stärkeren Zustand als eine LASIK. Das Entfernen von Hornhautgewebe mit einem Laser schwächt die Hornhaut. Bei der LASIK wird eine Lamelle in die Hornhaut geschnitten , die den tiefen Teil der Hornhaut freilegt. Diese Lamelle trägt lebenslang nicht mehr zur Festigkeit der Hornhaut bei. Anschliessend wird Gewebe weggelasert und dann die Lamelle neu positioniert. Bei der PRK trägt der Laser das Gewebe direkt an der Oberfläche der Hornhaut ab – es ist keine Lamelle erforderlich. Wir wissen aus unserer objektiven experimentellen Arbeit, die in renommierten, peer-reviewed Journalen veröffentlicht wurde, dass die Hornhaut dadurch stärker wird

Warum die Hornhautstärke wichtig ist

Eine der am meisten gefürchteten Komplikationen der refraktiven Laserchirurgie ist die “Ektasie”: hier wird die Hornhaut durch die Operation so stark geschwächt, dass sie sich krankhaft vorwölbt, was das Sehvermögen immer schlechter macht. In neunzig Prozent der Fälle ist eine LASIK dafür verantwortlich, und nicht eine PRK. Unsere Gruppe war massgeblich an der Entwicklung der einzigen wirksamen Behandlungsmethode der Ektasie beteiligt, dem sogenannten Cross-Linking der Hornhaut. Aber am Besten ist es, es gar nicht erst zu einer Ektasie kommen zu lassen. Wie Hippokrates schon sagte: “Zuallererst, nicht schaden…”.

Zusammengefasst sind alle Laserverfahren sicher, LASIK wie PRK. Das Risiko von dauerhaften Problemen (z.B. Sehbehinderung) ist sehr gering. Dennoch sind wir der Meinung, dass das Risiko immer minimiert werden sollte. Aus diesem Grund glauben wir, dass die trans-PRK für die meisten Patienten die beste Lösung ist, es sei denn, es gibt gute Gründe, ein anderes Verfahren durchzuführen. Die transPRK bietet die bestmögliche Sehkorrektur – zusammen mit der LASIK -, aber es lässt die Hornhaut – anders als die LASIK -für den Rest Ihres Lebens stärker zurück, so dass Sie eine stabile Korrektur mit einem geringeren Risiko für eine Ektasie erhalten.

Overall as a 50 year old, who wanted to continue to avoid wearing glasses and/or contact lenses, my goal was 100%... achieved with ELZA and Dr. Hafezi. I greatly appreciated the overall approach by Dr. Hafezi and his friendly and competent team in reviewing my vision at the start and following a comprehensive eye exam- the explanations of my options. For me I chose PRK in one eye for a monovision solution to correct short sightedness which was becoming more prevalent in night driving and sports such as skiing and cycling. PRK heals slowly but the outcome is worth the wait- and the cost- you use your eyes every waking moment- and for me I cannot see any reason not to pursue better vision with the ELZA team! I have been to the ELZA clinic to have surgery for my miopy, as my eyes were rejecting the contacts and I was tired of... wearing glasses. From the first preliminary visit, to the actual surgery and post-surgery care Dr. Hafezi and all his collaborators have been extremely professional, considerate and helpful. All the steps of the (very short!) surgery were thoroughly explained to me in order to be as calm as possible, together with the medication plan and precautions to be aware of so that my eyes could heal in the best way possible. My left eye had to be operated on because of a cataract. As I had an Artiflex lens implanted a few years ago, it had to... be removed first before a modern lens could be implanted to correct the cataract.
