Um exemplo de como isto pode ser dif\u00edcil \u00e9 mostrado num relat\u00f3rio de caso, publicado recentemente no Journal of Refractive Surgery Case Reports (1), em que um doente com Distrofia Muscular de Duchenne (DMD) necessitava de crom\u00f3foro fotoactivado para queratite infecciosa - CXL (PACK-CXL) para o tratamento de uma \u00falcera infecciosa da c\u00f3rnea.<\/p>\n<p class=\"p1\">As pessoas com DMD t\u00eam normalmente grandes dificuldades em levantar-se de uma posi\u00e7\u00e3o sentada ou deitada e em exibir <a href=\"https:\/\/en.wikipedia.org\/wiki\/Gowers%27s_sign\">Sinal de Gower<\/a> - incapacidade de se levantar devido \u00e0 fraqueza dos m\u00fasculos dos membros inferiores. Al\u00e9m disso, a maioria das pessoas com DMD desenvolve cardiomiopatia e insufici\u00eancia respirat\u00f3ria, o que pode resultar numa redu\u00e7\u00e3o da capacidade pulmonar quando os doentes alternam entre as posi\u00e7\u00f5es sentada e supina. Esta n\u00e3o \u00e9 a situa\u00e7\u00e3o ideal quando se efectua o cross-linking tradicional, em que o doente tem de ser transferido para uma cadeira reclin\u00e1vel e receber at\u00e9 30 minutos de irradia\u00e7\u00e3o UV enquanto est\u00e1 deitado de costas.<\/p>\n<p>No entanto, uma nova abordagem, a reticula\u00e7\u00e3o na l\u00e2mpada de fenda, iniciada por <a href=\"https:\/\/www.elza-institute.com\/pt\/sobre\/farhad-hafezi\/\">Prof. Farhad Hafezi<\/a> do <a href=\"https:\/\/www.elza-institute.com\/pt\/\">Instituto ELZA<\/a> em Zurique, permite que o CXL e o PACK-CXL sejam realizados com o doente sentado na vertical \u00e0 luz da l\u00e2mpada de fenda, utilizando um dispositivo de reticula\u00e7\u00e3o port\u00e1til, mont\u00e1vel na l\u00e2mpada de fenda, denominado <a href=\"https:\/\/www.emagine-eye.com\/c-eye-device\/\">C-Eye<\/a>. Esta abordagem foi utilizada pelo Dr. Boris Knyazer do Department of Ophthalmology, Soroka University Medical Center, Ben-Gurion University of the Negev, Beer-Sheva, Israel, para realizar com \u00eaxito dois procedimentos PACK-CXL num per\u00edodo de 5 dias, no olho de um doente de 39 anos, utilizador de cadeira de rodas e com DMD, para tratar uma queratite infecciosa da c\u00f3rnea. O procedimento foi simples: o doente simplesmente manobrou-se perante a l\u00e2mpada de fenda para receber a irradia\u00e7\u00e3o UV, evitando a necessidade de mudar de posi\u00e7\u00e3o ou o risco de comprometer a capacidade pulmonar do doente durante o per\u00edodo de irradia\u00e7\u00e3o UV.
