apresenta\u00e7\u00f5es:<\/span><\/p><p><strong><span data-preserver-spaces=\"true\">Individualiza\u00e7\u00e3o do CXL para ceratocone<\/span><\/strong><\/p><p><span data-preserver-spaces=\"true\">Nesta apresenta\u00e7\u00e3o, o Prof. Hafezi introduziu\u00a0<\/span><a class=\"editor-rtfLink\" href=\"https:\/\/www.elza-institute.com\/pt\/customized-crosslinking\/\" target=\"_blank\" rel=\"noopener\"><span data-preserver-spaces=\"true\">PACE (Phototherapeutic Keratectomy-Assisted Customized Epi-on Cross-linking)<\/span><\/a><span data-preserver-spaces=\"true\">PACE, um tratamento personalizado de segunda gera\u00e7\u00e3o para o ceratocone que tem mostrado resultados promissores. O PACE combina o cross-linking com um PTK guiado por um mapa epitelial para obter um aplanamento significativo da c\u00f3rnea. Ao contr\u00e1rio da t\u00e9cnica de primeira gera\u00e7\u00e3o, o PACE n\u00e3o requer um rastreador ocular durante a irradia\u00e7\u00e3o UV da c\u00f3rnea. \u00c9 efectuada uma pequena PTK orientada por um mapa epitelial sobre o cone, criando \u00e1reas epi-off e epi-on, resultando num gradiente de riboflavina entre elas. Este gradiente permite diferentes efic\u00e1cias de reticula\u00e7\u00e3o em diferentes \u00e1reas da c\u00f3rnea. Com quase o dobro do efeito de aplana\u00e7\u00e3o em compara\u00e7\u00e3o com a t\u00e9cnica de primeira gera\u00e7\u00e3o, o PACE \u00e9 particularmente eficaz para doentes m\u00edopes com valores da c\u00f3rnea na gama de 53-55 D. Pode considerar-se um ajuste fino subsequente com PRK guiado pela frente de onda depois de a topografia (forma) da c\u00f3rnea ter estabilizado. O aperfei\u00e7oamento cont\u00ednuo da t\u00e9cnica PACE tem como objetivo melhorar ainda mais os resultados, proporcionando melhores resultados e uma melhor vis\u00e3o aos doentes com ceratocone.<\/span><\/p><p><strong><span data-preserver-spaces=\"true\">Epi-on CXL de terceira gera\u00e7\u00e3o e Epi-on CXL de escrit\u00f3rio na l\u00e2mpada de fenda: Democratizar o acesso ao tratamento<\/span><\/strong><\/p><p><span data-preserver-spaces=\"true\">Nestas apresenta\u00e7\u00f5es, o Prof. Hafezi discutiu os dois principais desafios para garantir que o CXL epi-on na l\u00e2mpada de fenda funciona: garantir que a riboflavina suficiente passa atrav\u00e9s do epit\u00e9lio e chega ao estroma, e melhorar os protocolos de irradia\u00e7\u00e3o UV para compensar a redu\u00e7\u00e3o aproximada de 15% no componente essencial da rea\u00e7\u00e3o, o oxig\u00e9nio, no estroma quando o epit\u00e9lio \u00e9 deixado intacto. Al\u00e9m disso, explicou como o seu trabalho com os potenciadores de penetra\u00e7\u00e3o de riboflavina de segunda gera\u00e7\u00e3o e os protocolos de alta flu\u00eancia pulsada trouxeram agora um CXL epi-on eficaz para a l\u00e2mpada de fenda, aproximando o campo do objetivo de tornar o CXL mais acess\u00edvel a todos.<\/span><\/p><p><strong><span data-preserver-spaces=\"true\">CXL em c\u00f3rneas ultra-finas<\/span><\/strong><\/p><p><span data-preserver-spaces=\"true\">O Prof. Hafezi explicou aos cirurgi\u00f5es oftalmol\u00f3gicos reunidos o papel central (e limitador) que o oxig\u00e9nio desempenha no CXL no estroma da c\u00f3rnea. Este conhecimento permitiu aos cirurgi\u00f5es e investigadores da ELZA modelar toda a rea\u00e7\u00e3o dos raios UV, riboflavina, oxig\u00e9nio e tecido do estroma e permitiu-lhes desenvolver o protocolo sub400 para reticular com seguran\u00e7a c\u00f3rneas finas e ultrafinas, sem os inconvenientes dos protocolos tradicionais de CXL para c\u00f3rneas finas que normalmente envolvem o espessamento artificial da c\u00f3rnea, o que pode ter resultados imprevis\u00edveis.<\/span><\/p><p><span data-preserver-spaces=\"true\">Medindo a espessura da c\u00f3rnea e consultando o algoritmo sub400, os oftalmologistas podem determinar a dura\u00e7\u00e3o adequada da irradia\u00e7\u00e3o UV para a espessura medida, permitindo uma estrutura personalizada e fi\u00e1vel para o reticulado de c\u00f3rneas finas - tudo o resto permanece igual a qualquer outro reticulado do protocolo de Dresden!<\/span><strong style=\"text-align: var(--text-align); background-color: initial; color: var( promessa<\/span><\/strong><\/p><p><span data-preserver-spaces=\"true\">O Prof. Hafezi explicou como o crom\u00f3foro fotoactivado para a reticula\u00e7\u00e3o ceratite-c\u00f3rnea (PACK-CXL) para infec\u00e7\u00f5es da c\u00f3rnea evoluiu do protocolo de Dresden, protocolos de irradia\u00e7\u00e3o ultravioleta de baixa flu\u00eancia\/intensidade inferior CXL para protocolos mais r\u00e1pidos e acelerados de alta flu\u00eancia\/intensidade superior, e como isto proporciona um efeito de elimina\u00e7\u00e3o de agentes patog\u00e9nicos maior e mais profundo.<\/span><strong><span
