Utilizando c\u00f3rneas porcinas como um modelo ex vivo validado, os autores compararam os resultados biomec\u00e2nicos entre o seu novo protocolo epi-on e o protocolo epi-off acelerado amplamente adotado (9 mW\/cm2, 10 minutos, flu\u00eancia de 5,4 J\/cm2). Os resultados n\u00e3o demonstraram qualquer diferen\u00e7a significativa entre os m\u00f3dulos el\u00e1sticos do novo protocolo epi-on e da refer\u00eancia epi-off acelerada, embora ambos fossem significativamente mais r\u00edgidos do que os controlos n\u00e3o tratados. A barreira epitelial apresenta uma dupla limita\u00e7\u00e3o: a riboflavina, uma mol\u00e9cula hidrof\u00edlica, penetra mal no epit\u00e9lio intacto, enquanto o pr\u00f3prio epit\u00e9lio consome oxig\u00e9nio a taxas substancialmente superiores aos n\u00edveis do estroma. As tentativas anteriores, incluindo os intensificadores qu\u00edmicos de primeira gera\u00e7\u00e3o e a iontoforese, n\u00e3o conseguiram fornecer concentra\u00e7\u00f5es suficientes de riboflavina ou aumentaram a complexidade do procedimento sem resolver completamente estas limita\u00e7\u00f5es.<\/span><\/p><p>Ao resolver as limita\u00e7\u00f5es da riboflavina e do oxig\u00e9nio atrav\u00e9s do melhoramento qu\u00edmico e da dosagem optimizada de UV-A, o protocolo descrito por Lu et al. simplifica potencialmente o CXL epi-on, transformando-o num procedimento totalmente baseado no consult\u00f3rio, que pode evitar a necessidade de um ambiente cir\u00fargico est\u00e9ril.<\/p><p>No entanto, os autores reconhecem limita\u00e7\u00f5es cr\u00edticas. O modelo porcino ex vivo, embora metodologicamente robusto, n\u00e3o pode reproduzir totalmente as respostas in vivo, como a cicatriza\u00e7\u00e3o de feridas e a remodela\u00e7\u00e3o dos querat\u00f3citos. Al\u00e9m disso, as diferen\u00e7as anat\u00f3micas (em particular, o epit\u00e9lio porcino mais espesso e a aus\u00eancia da camada de Bowman) requerem uma an\u00e1lise cuidadosa quando se extrapolam os resultados para a pr\u00e1tica cl\u00ednica.<\/p><p>Embora estes resultados forne\u00e7am provas biomec\u00e2nicas convincentes, a valida\u00e7\u00e3o cl\u00ednica continua a ser essencial.
