Eis como a imagem da c\u00f3rnea est\u00e1 a aprender a detetar a doen\u00e7a antes que esta turve uma \u00fanica linha de vis\u00e3o.<\/p>\n\n<p>O queratocono \u00e9 muito mais f\u00e1cil de controlar quando \u00e9 detetado precocemente \u2014 antes que a c\u00f3rnea tenha afinado e protuberido o suficiente para turvar a vis\u00e3o. Se for detetado mais tarde, estar\u00e1 a gerir distor\u00e7\u00e3o, cicatrizes e, nos piores casos, um transplante de c\u00f3rnea. Se for detetado precocemente, um \u00fanico tratamento minimamente invasivo pode, muitas vezes, parar a doen\u00e7a nas suas pistas. A diferen\u00e7a entre esses dois futuros prende-se quase inteiramente \u00e0 imagiologia.<\/p>\n\n<p>A c\u00f3rnea mais dif\u00edcil de ler \u00e9 aquela que parece normal. Um cone na sua fase mais precoce \u2014 o chamado queratocono subcl\u00ednico ou fruste \u2014 pode passar num exame ocular padr\u00e3o, realizar uma topografia de rotina e ainda assim estar silenciosamente a progredir. Em lado nenhum isso importa mais do que na triagem de candidatos \u00e0 corre\u00e7\u00e3o da vis\u00e3o a laser, onde tratar um olho com uma fraqueza oculta o pode levar \u00e0 ectasia. Durante d\u00e9cadas o desafio tem sido o mesmo: como detetar um problema que ainda n\u00e3o alterou a forma do olho?<\/p>\n\n<h4>Medir rigidez, n\u00e3o apenas a forma<\/h4>\n\n<p>A resposta mais intrigante no horizonte n\u00e3o mede forma alguma. Mede rigidez.<\/p>\n\n<p>O ceratocone \u00e9, na sua ess\u00eancia, uma doen\u00e7a biomec\u00e2nica: a c\u00f3rnea enfraquece antes de se deformar. A topografia e a tomografia padr\u00e3o capturam a deforma\u00e7\u00e3o \u2013 o encurvamento e o afinamento \u2013, o que significa que, por defini\u00e7\u00e3o, est\u00e3o a ler a consequ\u00eancia e n\u00e3o a causa. A microscopia Brillouin vai diretamente \u00e0 causa. Utilizando um fen\u00f3meno chamado dispers\u00e3o de luz Brillouin, mede as propriedades el\u00e1sticas do tecido em pontos focais em toda a c\u00f3rnea, sem nunca tocar no olho, e constr\u00f3i um mapa tridimensional de qu\u00e3o r\u00edgida ou mole \u00e9 cada regi\u00e3o.<\/p>\n\n<p>No ceratocone em est\u00e1gio inicial, o mapa ilumina uma mancha suave focal: uma \u00e1rea enfraquecida rodeada por tecido que ainda parece, e mede, totalmente normal em exames convencionais. Num estudo amplamente discutido, liderado por J Bradley Randleman, a microscopia Brillouin com rastreamento de movimento separou olhos com ceratocone subcl\u00ednico de controlos saud\u00e1veis com precis\u00e3o perfeita - 100% de sensibilidade e especificidade - superando todas as m\u00e9tricas de Scheimpflug na mesma coorte. Nos mapas bidimensionais, a diferen\u00e7a era vis\u00edvel a olho nu.<\/p>\n\n<p>Vale a pena sermos claros sobre onde isto se situa. Os aparelhos ainda s\u00e3o maioritariamente feitos \u00e0 medida, as varreduras demoram minutos em vez de segundos, e os estudos at\u00e9 agora s\u00e3o pequenos. A microscopia de Brillouin ainda n\u00e3o est\u00e1 na sua cl\u00ednica local. Mas o princ\u00edpio por detr\u00e1s dela \u00e9 uma mudan\u00e7a genu\u00edna: imagem que relata a pr\u00f3pria fraqueza, n\u00e3o a altera\u00e7\u00e3o de forma que eventualmente se segue.
