No entanto, o tratamento requer uma l\u00e2mpada UV, gotas de riboflavina est\u00e9ril e um cirurgi\u00e3o treinado. Grande parte do mundo n\u00e3o disp\u00f5e de nenhum destes elementos. Essa lacuna tornou atraente uma ideia muito mais simples: trocar a l\u00e2mpada pelo sol e as gotas por um comprimido de vitamina B2. Um novo estudo do ELZA testou agora essa ideia em olhos vivos. Constatou-se que a riboflavina oral e a luz solar n\u00e3o endurecem minimamente a c\u00f3rnea.<\/p><p>O estudo foi liderado pelo primeiro autor, Dr. Emilio Torres-Netto, com o autor s\u00e9nior Prof. Farhad Hafezi. \u00c9 publicado na edi\u00e7\u00e3o de agosto de 2026 do Journal of Refractive Surgery.<\/p><h3>Porque \u00e9 que a reticula\u00e7\u00e3o por luz solar \u00e9 uma ideia t\u00e3o aliciante<\/h3><p>O CXL foi descritos pela primeira vez em 2003. Desde ent\u00e3o, tornou-se o tratamento padr\u00e3o para travar a progress\u00e3o do ceratocone. O mecanismo \u00e9 fotoqu\u00edmico. A riboflavina (vitamina B2) penetra no estroma corneal, absorve a luz ultravioleta e desencadeia uma rea\u00e7\u00e3o que endurece o tecido.<\/p><p>Como a rea\u00e7\u00e3o necessita tanto da vitamina como de uma fonte de UV, o acesso depende de equipamento e forma\u00e7\u00e3o. Permanece, portanto, limitado em locais com poucos recursos e em regi\u00f5es remotas. Em 2019, um pequeno relat\u00f3rio observacional dos Estados Unidos ofereceu uma alternativa. Sugeria que uma dose elevada de riboflavina oral, combinada com 15 minutos de sol direto por dia, poderia estabilizar o ceratocone. No entanto, esse relat\u00f3rio n\u00e3o apresentava quaisquer medi\u00e7\u00f5es biomec\u00e2nicas.<\/p><p>O trabalho de laborat\u00f3rio da pr\u00f3pria ELZA j\u00e1 tinha demonstrado que a luz solar pode cumprir essa fun\u00e7\u00e3o, em princ\u00edpio. Num estudo de 2023, c\u00f3rneas de su\u00ednos tiveram o seu epit\u00e9lio removido, foram embebidas em riboflavina e expostas \u00e0 luz solar. Endureceram cerca de tanto quanto endurecem com o CXL convencional. Ainda assim, a quest\u00e3o em aberto era se o mesmo efeito se mant\u00e9m num olho vivo, com um epit\u00e9lio intacto e uma vitamina ingerida.<\/p><h3>Como o estudo foi realizado<\/h3><p>A equipa realizou um estudo in vivo prospetivo e controlado em 16 coelhos macho da ra\u00e7a Nova Zel\u00e2ndia Branco (32 olhos). O protocolo foi aprovado pelo Gabinete Veterin\u00e1rio do Cant\u00e3o de Zurique. Num primeiro passo, quatro coelhos (8 olhos) receberam riboflavina por via oral a 6 mg\/kg por dia durante 14 dias. A microscopia de fluoresc\u00eancia de dois fot\u00f5es quantificou posteriormente a quantidade de riboflavina que tinha atingido o estroma.<\/p><p>No segundo passo, 12 coelhos (24 olhos) foram aleatorizados numa propor\u00e7\u00e3o de 1:1. Um grupo seguiu o protocolo de riboflavina oral e luz solar; o outro recebeu apenas luz solar. Os animais foram para o exterior durante 15 minutos diariamente at\u00e9 ser atingida uma dose acumulada de 2.700 klux\u00b7h. Esse valor corresponde \u00e0 exposi\u00e7\u00e3o relatada nas observa\u00e7\u00f5es em humanos. A rigidez corneal foi ent\u00e3o medida de duas formas: por extensometria uniaxial de tens\u00e3o-deforma\u00e7\u00e3o e por elastografia OCT. Esta \u00faltima mapeia o grau de deforma\u00e7\u00e3o do tecido sob uma pequena altera\u00e7\u00e3o de press\u00e3o.<\/p><h3>A riboflavina por via oral e a luz solar n\u00e3o provocaram endurecimento da c\u00f3rnea<\/h3><p>A extensometria n\u00e3o encontrou absolutamente nenhuma diferen\u00e7a entre os grupos. A tens\u00e3o a uma deforma\u00e7\u00e3o de 0,1 foi de 152 \u00b1 11,5 kPa nos olhos de controlo e de 146 \u00b1 7,0 kPa nos olhos tratados (p = 0,57). Da mesma forma, o m\u00f3dulo el\u00e1stico m\u00e9dio entre 0,1 e 0,2 de deforma\u00e7\u00e3o foi de 4,1 MPa versus 4,0 MPa (p = 0,870).<\/p><p>Em contrapartida, a elastografia por OCT apontava, quando muito, no sentido oposto. A deforma\u00e7\u00e3o na metade posterior da c\u00f3rnea foi significativamente superior nos olhos tratados em compara\u00e7\u00e3o com os controlos (5,51\u2030 versus 3,98\u2030, p = 0,039). Isso \u00e9 consistente com uma pequena redu\u00e7\u00e3o na rigidez em vez de um aumento. Os autores interpretam isto com cautela. Em primeiro lugar, a concentra\u00e7\u00e3o m\u00e9dia de riboflavina no estroma ap\u00f3s a administra\u00e7\u00e3o oral foi de apenas 0,000081%. Este valor \u00e9 cerca de 494 vezes inferior ao de 0,04% alcan\u00e7ado pelas gotas t\u00f3picas a uma profundidade de 400 \u00b5m no estroma. Em segundo lugar, a flu\u00eancia UV absorvida situa-se em cerca de 4,3 mJ\/cm\u00b2, cerca de 1 000 vezes inferior \u00e0 proporcionada pelo protocolo de Dresden. Al\u00e9m disso, o epit\u00e9lio intacto limita a penetra\u00e7\u00e3o dos raios UV e o fornecimento de oxig\u00e9nio do qual a rea\u00e7\u00e3o depende. Por outras palavras, o insucesso deve-se \u00e0 biodisponibilidade e n\u00e3o \u00e0 fotoqu\u00edmica.<\/p><h3>Por que \u00e9 que esta descoberta \u00e9 importante fora do laborat\u00f3rio<\/h3><p>Entretanto, v\u00e1rias iniciativas comerciais promovem a riboflavina por via oral e a exposi\u00e7\u00e3o \u00e0 luz solar como tratamento para o ceratocone. O ceratocone progride quando n\u00e3o \u00e9 tratado, pelo que um substituto ineficaz n\u00e3o \u00e9 uma op\u00e7\u00e3o neutra. Pode atrasar o in\u00edcio da terapia estabelecida o tempo suficiente para que ocorra uma perda irrevers\u00edvel da vis\u00e3o.<\/p><p>Ainda assim, os autores t\u00eam o cuidado de especificar o que os seus dados cobrem e o que n\u00e3o cobrem. Testaram um efeito de *cross-linking* isolado. O estudo n\u00e3o aborda a suplementa\u00e7\u00e3o nutricional, a defici\u00eancia de riboflavina nem qualquer papel antioxidante na doen\u00e7a. O *cross-linking* simplificado continua tamb\u00e9m a ser um objetivo que vale a pena perseguir, particularmente onde o CXL convencional est\u00e1 fora do alcance. O caminho at\u00e9 l\u00e1, sugerem os autores, passa por uma melhor administra\u00e7\u00e3o t\u00f3pica e por fontes de luz ajustadas ao espetro de absor\u00e7\u00e3o da riboflavina.<\/p><h3>Acerca do estudo<\/h3><p>Este trabalho foi apoiado em parte pela Light for Sight Foundation (Dietikon, Su\u00ed\u00e7a) e pela VELUX Stiftung (Zurique, Su\u00ed\u00e7a).
