We are honored that you have chosen ELZA Institute for your eye care. To ensure your visit is comfortable, efficient, and personalized, we kindly ask for your cooperation with the information below.

Our team is committed to providing timely care, especially for urgent concerns, while keeping waiting times as short as possible. We truly look forward to welcoming you and caring for your vision.

Health Insurance Card

  • Please remember to bring your valid health insurance card to every appointment.
  • This allows us to process your visit smoothly and without delay.

Patient Registration

  • Before your first appointment, we kindly ask that you complete our Patient Registration Form.
  • You can fill out the form online (available in German, English, and French; change the language in the upper right corner of the first page) or download it as a PDF (German only), print it out, and send it to us in advance by email or post.
  • As soon as we receive your completed registration, a member of our team will reach out to you to schedule your appointment at a convenient time.
  • Please note that we are only able to schedule appointments after receiving your completed registration form.
  • Submitting your information in advance helps us prepare thoughtfully for your visit and provide you with the highest level of individualized care.

Medical Records

To better understand your eye health history, please bring any relevant records, including:

  • Previous eyeglass or contact lens prescriptions
  • Reports from other ophthalmologists or eye clinics

These documents allow us to offer a thorough and tailored evaluation.

Billing Information

Consultation fees are billed directly to your health insurance provider.

If you prefer not to receive a copy of your invoice, or if you would like the invoice sent directly to you, please let us know.

Appointment Cancellations & Missed Visits

If you are unable to attend your scheduled appointment, we kindly ask that you notify us at least 24 hours in advance.

You may cancel:

Appointments not canceled at least 24 hours in advance will incur an administrative fee of CHF 100.

For scheduled treatments involving self-pay services, 50% of the agreed treatment cost will be charged. These fees are not covered by health insurance.

For outstanding balances, reminder fees apply as follows:

  • Second reminder: CHF 30
  • Third reminder: CHF 50

We greatly appreciate your understanding and your consideration of our time and scheduling. Your cooperation allows us to provide attentive care to all of our patients.

General

Are you coming for a second opinion? If you have any medical reports about your eye disease, then please bring these with you.

We may need to dilate your pupils during consultations using special eyedrops. If this is the case, then please take this into consideration when planning your trip to ELZA.

Laser eye surgery

Please do not wear contact lenses for at least 2 weeks before the initial consultation. Contact lenses can change the shape of the cornea, and it takes time for the cornea to return to its natural shape – which is how we need the cornea to be to perform accurate measurements of it. If contact lenses are worn before your assessment, we will obtain false readings, and we won’t be able to tell you with certainty if and how your eyes could be lasered.

A precise preliminary examination is only possible if you have followed the 2 weeks without contact lenses.

Keratoconus

Just like with laser refractive surgery, please do not wear contact lenses for at least 2 weeks before your assessment – the change in corneal shape that contact lenses can cause can seriously distort the examination and our doctors will not be able to provide you with certainty about a possible progression of the disease.

In addition, a clean (undistorted) pre cross-linking measurement is important so that we can determine whether keratoconus progression has actually come to a standstill after cross-linking.

So please do not wear the contact lenses – just one hour of wearing them strongly deforms the cornea and renders our measurements inaccurate. We are aware that this is a long time, especially for young working people, but two weeks is the absolute bare minimum.

Please also bring old documents, if any. All older documents are valuable and should be collected if possible and brought to the investigation: old prescription glasses, old corneal topographies, even old lenses.

Eyelids/ Orbital surgery

Please bring old portrait photos. Our doctors want to know what your eyelids, eyebrows or orbit looked like 10, 20 and 30 years ago.

Ideally you are between 20 and 40 years of age in the pictures.

Portrait photos are ideal because you are looking straight into the camera and your head is not tilted or turned to one side.

Old travel passes or passports are sufficient: the head position on passport photos is perfect for our purposes.

Glaucoma/Retina

We use advanced imaging technology that often eliminates the need for pupil dilation.
However, dilation may still be necessary, so we kindly ask that you avoid driving yourself
to your appointment.
