In den Oberlidern und Unterlidern liegen jeweils mehrere Dutzend Talgdrüsen, die den Fettanteil des Tränenfilms bilden. Sind diese Drüsen aufgrund einer chronischen Entzündung verstopft, dann ist der Tränenfilm von seiner Qulität her schlecht.

Die Oberfläche des Auges ist dann chronisch trocken. Dies wird vom Gehirn registriert, welches der Tränendrüse den Befehl gibt: “Produziere mehr Wasser”. Und so kommt es, dass paradoxerweise ein chronisch Trockenes Auge sehr häufig intervallartig tränt.

Stadien der Blepharitis

In frühen Stadien zeigen sich “Pfropfen” auf der Lidkante, die die Öffnungen der Talgdrüsen verstopfen. Eine einfache mechanische Reinigung kann hier effizient helfen.

Wird die Lidrandentzündung jedoch jahrelang nicht behandelt, dann verändert sich das Lid anatomisch. Die Lidkante rundet immer mehr ab und ist nicht mehr in der Lage, den Tränen film auf der Oberfläche des Auges zu verteilen, ähnlich einem Scheibenwischer mit Scharten auf der Wischfläche.

Seltene Gründe für ein Trockenes Auge

Daneben können aber auch allgemeine Erkrankungen, wie zB die Schuppenflechte (Rosacea), Diabetes, Polyarthritis, Lupus erythematodes und das Sjögren-Syndrom trockene Augen bewirken. Hormonelle Ursachen sind der Hormonersatz in den Wechseljahren und die Anti-Baby-Pille. Auch ein sogenannter Morbus Crohn und die Colitis Ulcerosa können zu einem trockenen Auge führen, dies wegen der starken Durchfälle und der Malabsorption.

Lebensqualität

Obschon in den meisten Fällen nicht gefährlich, ist die Lebensqualität der Betroffenen zum Teil stark eingeschränkt.

Die Beschwerden bei trockenem Auge sind vielfältig und reichen von gering bis massiv: Augenbrennen und -beissen, Verschwommensehen, Doppelsehen, Kopfschmerzen. Typischerweise treten diese Beschwerden nach längerer Bildschirmarbeit, Fernsehen oder Lesen gehäuft auf.

Bildschirmberufe und KontaktlinsenträgerInnen

Trockene Augen sind weltweit einer der Hauptgründe dafür, dass Patienten den Augenarzt aufsuchen. Vermehrte Bildschirmarbeit und klimatisierte Arbeitsplätze haben dieses Problem in den letzten Jahrzehnten nochmals deutlich verschärft.

Die Zusammensetzung des Tränenfilms, das Zusammenspiel von Bindehaut und Hornhaut und die verstärkte Verdunstung des Tränenfilms spielen eine wichtige Rolle beim Entstehen eines Trockenen Auges. Diese Veränderungen können mit relativ einfachen Mitteln (Schirmer-Test, BUT, “break up time”) festgestellt werden und dann mit modernen Methoden vertieft untersucht werden.

In der Schweiz sind ca. 600’000 Personen vom sogenannten „Dry Eye-Syndrom“, “Syndrom des Trockenen Auges” oder auch “Sicca-Syndrom” genannt, betroffen. Dies sind die offiziellen Zahlen und die Dunkelziffer dürfte weitaus höher liegen. Ein weiterer, häufig gebrauchter Begriff ist das “Office Eye”. Früher wurde als einzige Therapie künstliche Tränen angeboten, heutzutage gibt es eine Vielzahl weiterer Optionen.

In Switzerland, it’s reported that 600,000 people have been officially diagnosed with DES – but it’s likely that the true number is much higher thanks to unreported cases – people who live with DES, but haven’t asked for help.

Nachtarbeit

Trockene Augen betreffen grundsätzlich Personen jeden Alters, wobei sich eine Häufung bei folgenden Bevölkerungsgruppen zeigt:

  • Träger von Kontaktlinsen (vor allem weiche Kontaktlinsen)
  • Berufstätige, welche viel an Computern arbeiten und/oder sich häufig in klimatisierten Räumen aufhalten
  • Ältere Menschen (>65 Jahre). Mit zunehmendem Alter nimmt das Trockene Auge zu.
  • Nachtarbeiter / Schichtarbeiter, da die Tränenproduktion eng an den Tag-Nacht-Rhythmus gekoppelt ist.

Overall as a 50 year old, who wanted to continue to avoid wearing glasses and/or contact lenses, my goal was 100%... achieved with ELZA and Dr. Hafezi. I greatly appreciated the overall approach by Dr. Hafezi and his friendly and competent team in reviewing my vision at the start and following a comprehensive eye exam- the explanations of my options. For me I chose PRK in one eye for a monovision solution to correct short sightedness which was becoming more prevalent in night driving and sports such as skiing and cycling. PRK heals slowly but the outcome is worth the wait- and the cost- you use your eyes every waking moment- and for me I cannot see any reason not to pursue better vision with the ELZA team! I have been to the ELZA clinic to have surgery for my miopy, as my eyes were rejecting the contacts and I was tired of... wearing glasses. From the first preliminary visit, to the actual surgery and post-surgery care Dr. Hafezi and all his collaborators have been extremely professional, considerate and helpful. All the steps of the (very short!) surgery were thoroughly explained to me in order to be as calm as possible, together with the medication plan and precautions to be aware of so that my eyes could heal in the best way possible. My left eye had to be operated on because of a cataract. As I had an Artiflex lens implanted a few years ago, it had to... be removed first before a modern lens could be implanted to correct the cataract.
