PACK-Crosslinking bei Keratitis

U-Licht und Vitamin B₂

Der Gebrauch von ultraviolettem Licht und Vitamin B2 (Riboflavin) zur Sterilisierung von Oberflächen und Lösungen ist nicht neu. Diese Kombination wird seit Jahrzehnten in der Medizin und der chemischen Industrie angewandt. So wird zum Beispiel die Anzahl Mikroben in grossen olympischen Schwimmbecken häufig mit Vitamin B2 und Sonnenlicht reduziert. Daneben wird die Technologie seit Jahren eingesetzt, um die Anzahl an Bakterien in Bluttransfusionenzu reduzieren. Daneben wurde auch die SODIS (Solar Disinfection)-Technologie von der ETH Zürich entwickelt, um die Belastung von Trinkwasser mit Mikroben zu verringern. Hier wird Vitmain B₂ zu Wasser in PET Flaschen gegeben und diese dann während mehrerer Stunden direktem Sonnenlicht ausgesetzt. Nach dieser Behandlung sind Bakterien, Pilze und sogar Viren in der Flasche abgetötet.

UV-Licht und Vitamin B₂ sind die Bestandteile des Cross-Linking der Hornhaut, welches seit 2002 zur Behandlung des Keratokonus eingesetzt wird und welches wir maßgeblich mitentwickelt haben. Es stellte sich also die Frage, ob die Methode eventuell auch bei Infekten der Hornhaut eingesetzt werden kann.

Cross-Linking bei Hornhautinfekten: Vorreiter Zürich

In den Jahren 2006 und 2007 hat unsere Gruppe in Zürich die weltweit ersten Patienten mit dieser neuen Methode behandelt. Es wurden nur Patienten behandelt, die nach Behandlung mit Antibiotika keinerlei Verbesserung zeigen. Der Erfolg war vielversprechend und der Anfang eines Forschungszweiges, der immer mehr an Bedeutung gewinnt.

   

PACK-Cross-Linking (PACK-CXL)

Am internationalen Cross-Linking Kongress in Zürich im Jahr 2014 schlugen Farhad und Nikki Hafezi einen neuen Begriff vor, der das Cross-Linking für Infekte besser beschreiben soll: PACK-CXL (photoactivated chromophore for infectious keratitis). Der Vorschlag wurde angenommen und seither für diese neue Behandlungsmethode benutzt.

Gründe, um PACK-CXL einzusetzen

Es gibt eine Reihe von Gründen, diese neue Methode einzusetzen:

  • Gemischte Infekte:
    in vielen Ländern der Welt sind die Hornhautinfekte nicht bakteriell, sondern gemischt, das heisst durch Bakterien und Pilze bedingt. Antibiotika alleine helfen hier nicht. PACK-Cross-Linking tötet Bakterien und Pilze ab.
  • Therapeutisches und diagnostisches Dilemma: zu Beginn einer Infektion ist es manchmal sehr schwierig,abzuschätzen, ob die Infektion durch Bakterien oder durch Pilze verursacht wird. Dementsprechend schwierig ist es, die geeignete Therapie auszuwählen. PACK-Cross-Linking vereinfacht den Ansatz, weil es gleichzeitig Bakterien und Pilze abtötet.
  • Finanzielle Belastung: in vielen Ländern der Welt kann die Hornhautinfektion aus finanziellen Gründen nicht erfolgreich behandelt werden. Dabei entstehen die Kosten weniger durch die Medikamente als durch die vielen Arztbesuche, die nötig sind. Ein einmaliges PACK-Cross-Linking reduziert die Gesamtanzahl von Arztbesuchen, die nötig sind.

Wie wirkt PACK-CXL?

Es wirkt auf drei verschiedene Weisen:

1) Es verhindert die Vermehrung von Mikroorganismen, indem es die Erbsubstanz deaktiviert,
2) Es tötet Mikroorganismen direkt durch einen sehr hohen oxidativen Stress und
3) Es erhöht den Widerstand des Gewebes gegenüber der entzündlichen Verdauung.

PACK-CXL Klinische Studien

Im Jahr 2021 wurde eine große, internationale, multizentrische Phase-III-Studie veröffentlicht , an der Patienten mit einer infektiösen Keratitis teilnahmen, die vermutlich durch Bakterien, Pilze oder eine Mischung aus beidem verursacht wurde. Die Patienten wurden entweder mit PACK-CXL oder mit einer antimikrobiellen Standardtherapie behandelt. Die Behandlung wurde als erfolgreich definiert, wenn jegliche Anzeichen einer Infektion verschwunden waren und die Hornhaut reepithelisiert war (d. h. die Zellen an der Oberfläche der Hornhaut über der Infektionsstelle wieder nachgewachsen waren).Das Ergebnis: Beide Behandlungen waren zu etwa 90 % erfolgreich, und bei der Zeit bis zur Reepithelisierung gab es keinen Unterschied zwischen den Behandlungsgruppen.

The article by Lu et al. reports an in vitro study, conducted to determine whether high-fluence PACK-CXL can be accelerated while still maintaining its antibacterial efficacy. The study found that high-fluence PACK-CXL did indeed decrease the bacterial concentration of several clinically significant bacterial strains, including S. aureus, P. aeruginosa, and S. epidermidis. These strains are commonly implicated in bacterial keratitis and contact lens-associated keratitis. Furthermore, the authors found that higher total fluence PACK-CXL protocols led to a corresponding increase in bacterial killing ratio (BKR).

Overall as a 50 year old, who wanted to continue to avoid wearing glasses and/or contact lenses, my goal was 100%... achieved with ELZA and Dr. Hafezi. I greatly appreciated the overall approach by Dr. Hafezi and his friendly and competent team in reviewing my vision at the start and following a comprehensive eye exam- the explanations of my options. For me I chose PRK in one eye for a monovision solution to correct short sightedness which was becoming more prevalent in night driving and sports such as skiing and cycling. PRK heals slowly but the outcome is worth the wait- and the cost- you use your eyes every waking moment- and for me I cannot see any reason not to pursue better vision with the ELZA team! I have been to the ELZA clinic to have surgery for my miopy, as my eyes were rejecting the contacts and I was tired of... wearing glasses. From the first preliminary visit, to the actual surgery and post-surgery care Dr. Hafezi and all his collaborators have been extremely professional, considerate and helpful. All the steps of the (very short!) surgery were thoroughly explained to me in order to be as calm as possible, together with the medication plan and precautions to be aware of so that my eyes could heal in the best way possible. My left eye had to be operated on because of a cataract. As I had an Artiflex lens implanted a few years ago, it had to... be removed first before a modern lens could be implanted to correct the cataract.
