sub400: extrem dünne Hornhäute cross-linken

Das ursprüngliche CXL-Protokoll erlaubt eine sichere Behandlung, wenn die Dicke der Hornhaut bei 400 μm oder höher liegt. Doch in fortgeschrittenen Fällen von Keratokonus, pelluzider marginaler Degeneration (PMD) und Ektasie nach LASIK kann die Hornhaut dünner sein.

Um eine solche Hornhaut trotzdem zu behandeln, wird hypo-osmolares Riboflavin benutzt. Dieses spezielle Vitamin B veranlasst die Hornhaut die Flüssigkeit zu absorbieren, wodurch sie anschwillt. Diese Technik wurde 2007 entwickelt und 2009 publiziert, mit Prof. Hafezi als erster Autor (1). Seither gilt diese Anwendung ein weltweiter Standard.

2008: “Geburt” des CXL in ultradünnen Hornhäuten.

Zürich, Schweiz: Ein 26 Jahre alter Patient wird mittels epi-off CXL behandelt. Nach Entfernung des Epithels und der Anwendung von „normalem“ iso- osmolarem Riboflavin beträgt die Hornhautdicke 325 Mikrometer; zu wenig, um ein sicheres CXL durchzuführen. In einem zweiten Schritt verwendeten wir hypo-osmolares Riboflavin. Nach 10 weiteren Minuten war die Hornhaut auf 407 Mikrometer „angeschwollen“, und das CXL konnte sicher durchgeführt werden.

Die Behandlung von extrem dünnen Hornhäuten erfordert eine große Erfahrung, da viele Faktoren berücksichtigt werden müssen. Die Häufigkeit des Tropfens des Vitamin B2, die Anwendung eines Lidspanners, all diese Elemente können die Hornhaut unter Umständen stark ausdünnen, so dass eine Behandlung nicht mehr möglich ist. Unsere Erfahrung ist eine der längsten weltweit und erlaubt es uns, auch noch bei (für andere zu) dünnen Hornhäuten ein CXL auf sichere Weise durchzuführen.

Die Zukunft ist jetzt - das sub400 Protokoll

Im Laufe der Jahre wurden mehrere Strategien zur Behandlung sehr dünner Hornhäute entwickelt. Der medizinische Direktor von ELZA, Farhad Hafezi, hat dazu beigetragen, die Wirksamkeit dieser Techniken klinisch und experimentell zu überprüfen (siehe unten). Dazu gehören das Anschwellen dünner Hornhäute auf eine sichere Dicke für das Cross-Linking (wie oben beschrieben) (1), das Kontaktlinsen-unterstützte CXL (CACXL) (2) und die Epithelial-Island Technik (3), bei der eine Insel von Epithelzellen über der dünnsten Stelle der Hornhaut belassen wird.

Welche Technik gewählt wird, muss von Fall zu Fall sorgfältig abgewogen werden. Die Schwellung der Hornhaut kann unvorhersehbar sein, da die Hornhaut bei jedem Patienten in unterschiedlichem Maße anschwillt. CACXL reduziert den Verstärkungseffekt um 30 %, und Epithelinseln führen zu unvorhersehbaren Versteifungseffekten.

Die Mitarbeitenden der Klinik und der Forschung von ELZA haben unter der Leitung von Professor Hafezi eine neue Technik zum Cross-Linking dünner Hornhäute entwickelt, die auf die individuelle Hornhautdicke jedes Patienten zugeschnitten ist (4). Anstatt die Hornhaut zu verändern, passen wir die Lichtdosis an, um ein sicheres und wirksames Cross-Linking auch bei ultradünnen Hornhäuten zu erreichen. 

Unsere Forscher entdeckten die wesentliche Rolle von Sauerstoff beim Cross-Linking (5) und modellierten eine Formel, wie Riboflavin, UV-Licht und Sauerstoff in der Hornhaut interagieren, um sie während einer Zeitdauer vernetzen. Daraus entstand ein Algorithmus, der klinisch vollständig validiert ist (6) und der die Tiefe des Cross-Linkings vorhersagt. Das bedeutet, dass Chirurgen die Dauer der UV-Bestrahlung individuell anpassen können, um eine sichere Tiefe des Cross-Linkings zu erreichen, selbst bei Hornhäuten mit einer Dicke von 220 µm.

Dieses Protokoll, namens sub400, erlaubt es dem ELZA Institut ultradünne Hornhäute zu behandeln, ohne auf Methoden wie die künstliche Verdickung der Hornhaut durch Schwellung, das Belassen von Epithelinseln oder die Notwendigkeit einer Kontaktlinse zurückgreifen zu müssen. Dadurch werden die mit diesen Methoden verbundenen Nachteile (d.h. unvorhersehbare Wirkungen und suboptimale Hornhautverstärkung) vermieden.

Im Jahr 2021 wurde das sub400-Protokoll in der Zeitschrift American Journal of Ophthalmology (4) publiziert und hat eine Erfolgsquote von fast 90 %.

Die Forschungsarbeit von Dr. Lu konzentriert sich auf ein neues System zur Einstufung von Keratokonus, einer Erkrankung der Hornhaut, und hat Auswirkungen auf dessen Diagnose und Behandlung.
Prof. Farhad Hafezi, MD, PhD, FARVO ist der medizinische Direktor des ELZA-Instituts und wurde kürzlich von Ocular Surgery News (OSN) interviewt, wo er über die fortschrittliche CXL-Technologie zur Behandlung von Hornhautektasien wie Keratokonus sprach. In dem Interview wurden drei Hauptthemen behandelt.
Das ELZA Chirurgen- und Forscherteam war anwesend, und heute hielt Prof. Hafezi einen Vortrag zum Thema "Wie man dünne Hornhäute behandelt", in dem er auf die Geschichte des Cross-Linkings (CXL) zur Behandlung von Hornhautektasien wie Keratokonus einging. Anschließend brachte er die Delegierten auf den neuesten Stand der Forschung, die im Laufe der Jahre durchgeführt wurde, um das Cross-Linking dünner Hornhäute zu verbessern. Schlussendlich demonstrierte Prof. Hafezi die Ergebnisse des neuesten, von ELZA entwickelten Cross-Linking Protokolls für dünne Hornhäute, das sub400, das den Prozess vereinfacht und es Chirurgen erstmals ermöglicht, Hornhäute mit einer Dicke von 200 µm zu vernetzen.

Overall as a 50 year old, who wanted to continue to avoid wearing glasses and/or contact lenses, my goal was 100%... achieved with ELZA and Dr. Hafezi. I greatly appreciated the overall approach by Dr. Hafezi and his friendly and competent team in reviewing my vision at the start and following a comprehensive eye exam- the explanations of my options. For me I chose PRK in one eye for a monovision solution to correct short sightedness which was becoming more prevalent in night driving and sports such as skiing and cycling. PRK heals slowly but the outcome is worth the wait- and the cost- you use your eyes every waking moment- and for me I cannot see any reason not to pursue better vision with the ELZA team! I have been to the ELZA clinic to have surgery for my miopy, as my eyes were rejecting the contacts and I was tired of... wearing glasses. From the first preliminary visit, to the actual surgery and post-surgery care Dr. Hafezi and all his collaborators have been extremely professional, considerate and helpful.
